Alcohol is one of the few substances where stopping suddenly can be fatal. The most dangerous window is the first 72 hours, when withdrawal symptoms build, seizures are most likely, and delirium tremens (DT) can set in.

Roughly half of people with alcohol dependence experience withdrawal when they cut back, and a smaller but serious share develop life-threatening complications [1]. That is the whole reason medically supervised detox exists.

Here is what actually happens across those first three days, why the timeline unfolds the way it does, and why medical detox exists specifically for this window.

How Common Are Serious Withdrawal Symptoms?

About half of people with alcohol use disorder (AUD) experience withdrawal symptoms when they reduce or stop drinking [1]. Roughly 3% to 5% of people with alcohol use disorder develop delirium tremens, the most severe form of withdrawal; in untreated cases, delirium tremens has historically carried a mortality rate as high as 15% [2].

The danger of seizures and severe withdrawal is real, and it is also largely preventable with the right care. Proper treatment does not just make withdrawal easier. It changes the odds.

Why the First 72 Hours Are the Most Dangerous

Alcohol leaves the body relatively quickly, which means withdrawal starts sooner than people expect and escalates over the first few days rather than easing. The two most serious complications, withdrawal seizures and delirium tremens, both cluster inside this 72-hour window. After it, for most people, the physical danger begins to recede even as discomfort lingers.

Here is how those three days typically unfold. Timelines vary based on how much and how long someone has been drinking, their overall health, prior withdrawal history, and other factors, so treat this as the general shape of the first 72 hours of withdrawal, rather than a personal prediction.

Early symptoms can begin as soon as six hours after the last drink, sometimes while blood alcohol is still dropping. This stage is often mistaken for a bad hangover. What usually appears first are:

  • Anxiety and restlessness
  • Hand tremors, the classic “shakes”
  • Sweating
  • Nausea and loss of appetite
  • Headache
  • Trouble sleeping
  • A racing or pounding heart

For someone with mild dependence, this may be close to the worst of it. For someone with heavier, longer-term use, this is the opening stretch of something bigger.

Hours 12 to 24

As the first day progresses, symptoms intensify, and this is when the first serious complications can appear. Some people begin experiencing alcohol hallucinosis, which involves seeing, hearing, or feeling things that are not there.

This is frightening, but importantly, it is distinct from delirium tremens; the person generally still knows where they are and remains oriented. Their sense of reality is intact even as their senses misfire [3].

Withdrawal seizures also become a real risk during this window. Most alcohol withdrawal seizures occur between 6 and 48 hours after the last drink. They are typically generalized seizures; they can happen without warning, and for someone alone at home, they are one of the most dangerous risks.

Hours 24 to 48

This is often the most intense stretch of the entire process, known as the “peak.” Physical symptoms tend to be most intense here, which include: 

  • Heavy sweating
  • Elevated heart rate and blood pressure
  • Tremors, agitation, insomnia, and profound discomfort

Seizure risk remains elevated through this period. This is also the window where the body is under the most cardiovascular strain and is a large part of why medical monitoring during detox is not just a formality. Blood pressure and heart rate are watched precisely because this is when they can spike.

Hours 48 to 72

Delirium tremens, the most severe form of alcohol withdrawal, typically emerges 48 to 72 hours after the last drink, though it can appear later. DTs is a medical emergency, and it looks fundamentally different from earlier symptoms. It involves:

  • Severe confusion and disorientation
  • Extreme agitation
  • Vivid hallucinations
  • Fever
  • Heavy sweating
  • Dangerous swings in heart rate and blood pressure

Delirium tremens involves a genuine break from reality; the person does not know where they are or what is happening. This is the complication that carries real mortality risk, and it is precisely why the highest-risk individuals need to be somewhere with medical staff and monitoring [4].

Medically supervised detox is built specifically for this window. In a clinical setting, the same 72 hours look very different:

Medication to prevent the worst outcomes, such as benzodiazepines, is the standard of care, and they work. In one landmark trial, appropriate medication reduced the combined risk of seizures and delirium from the 10-16% range down to around 2%. That is the difference supervision makes, quantified [5].

Continuous monitoring of heart rate, blood pressure, and symptoms, using structured tools, catches escalating cardiovascular risk early rather than discovering it too late.

Rapid response if a seizure or DTs begin, in a place equipped to handle them.

Fluids, thiamine, and nutritional support addressing the deficiencies common in long-term alcohol use that can themselves cause serious harm.

Detox clears the alcohol from the body safely. It does not treat alcohol use disorder on its own, which is why detox is the beginning of treatment rather than the whole of it. But it is the step that makes everything after it possible.
